Dear friend,
We would like your voluntary participation in a confidential survey research study. You are being asked to participate in this research study because of your expertise or interest in household emergency preparedness.
It will take you about 20 minutes to participate in this study.
To participate, you must be 21 years of age or older and able to read English fluently.
The purpose of this research study is to field test the newly updated Household Emergency Preparedness Instrument (Heagele et al., 2020; Heagele et al., 2022) to determine the average time it takes to complete the 80 questions and to ask for feedback on the experience of taking the questionnaire.
If you agree to participate, you will be asked to answer one internet-based survey. You will answer the Household Emergency Preparedness Instrument questions as if you are a respondent for a study, along with a few demographic questions. You will then be asked about your experience with taking the survey and you will have at least one open-ended question to tell us if we missed anything, if something was difficult to understand, or if you have any recommendations for us to consider.
All participants will have free access to the final instrument for research and educational use. We will send you the instrument, scoring guide, and permission to use the HEPI once the study concludes. We hope to inspire you to use this instrument in your diverse communities of interest, especially as a measurement of change in pre- and post- intervention studies and longitudinal studies evaluating the outcomes of adequate household emergency preparedness.
If you know of anyone who may also like to participate in this study, you may forward this email to or share the attached flyer with your colleague. To mitigate any undue influence, please limit your email to the original recruitment and consent message.
If you have any questions, comments or concerns about the research, you can talk to:

Principal Investigator
