Ask Before You Act: The Power of Permission in Patient Care

In the rush of clinical care, it’s easy to forget that a patient’s body is not just a clinical site-it’s their personal space. As medical providers, we often touch patients during exams, procedures, or assessments without pausing to ask permission first. But that small moment of respect-asking “Is it okay if I touch you?”-can make a profound difference.

Asking for permission to touch isn’t just about courtesy-it’s about consent, dignity, and trauma-informed care. Many patients have lived through experiences we may never know about. For them, unexpected or unexplained touch can trigger anxiety, fear, or retraumatization. Even in emergencies, when time is limited, a quick, calm “I’m going to check your pulse now-is that okay?” can go a long way.

When we ask before we act:

  • We build trust.
  • We preserve autonomy.
  • We empower patients in a system where they often feel powerless.

Touch is powerful. When used with consent, it can be healing. Without consent, it can be harmful.

Let’s normalize consent in clinical care-not just for invasive procedures, but for every single patient interaction.

I find myself asking this even when not providing patient care. When positioning people for a photo, for example. It does take them off guard, but it also sets a precedent that they then use.
